DEVT_USB_RESET while a DEPSTRTXFER is still in flight on a user endpoint (kStarting*, no

resource id yet). The graceful cancel can only be issued once the start event delivers the

resource id, so the endpoint goes kStarting* -> kPendingCancel -> (DEPEVT_XFER_STARTED ->

DEPENDXFER) -> kCanceling -> (DEPEVT_CMD_CMPLT) -> kIdle, and Ep0Start() must stay parked behind

the idle barrier across both events.
